Responsibilities include:

• Perform scheduled assessment visits for new and existing patients in their homes to ensure that the care they are receiving is appropriate for their needs
• Communicate with the physician and address any issues or complaints from the patients and their families
• Provide ongoing education and in-services for the caregivers
• Manage visits on the field and prioritize scheduled visits based on the needs of each patient
• Participate in supervision visits for caregivers in the field as assigned
• Be the liaison between the patient, caregiver and True Care staff
• Perform Start of Care/ Initial patient home assessments to accurately assess a patient’s health care status, physical and psychosocial needs
• Confer with the patient's physician in ascertaining the need for home care and in developing, implementing and revising the Plan of Treatment
• Coordinate with caregiver as necessary regarding plan of care for each client
  

Job requirements:

• NYS License & currently registered as a Registered Professional Nurse/ RN
• Graduated from an approved school of nursing
• Home Care experience is a plus
• Must have a vehicle to travel between cases
