Our Community Nursing and Rapid response teams are seeking registered Assistant Practitioners, Nurses and Senior Nurses to join our team and work across our unique Integrated Acute, Community and Primary Care services to help us make a difference to our patients who require care and treatment in their own homes.
We provide a range of care and treatment to predominantly housebound patients who often have complex care needs or long term conditions that require treatment and support in wound care, catheter maintenance, palliative care, supporting people to die at home, insulin and diabetes care and a whole range of other health conditions.
The post holder will be responsible for the delivery of effective and high quality nursing care for patients in their own homes and other community settings at all levels. You will also plan and Implement treatment and care to patients with professional guidelines (Please see attached job descriptions for in depth description)

Our service operates over a combination of days, weekend working, twilights and occasional night shifts. The weekend hours range between 8am to 18.00 and our twilight service runs 7 days per week between the hours of 17:00 to 22:00. We offer our night nursing service between the hours of 22.00 and 08:00, 7 days per week. The successful candidate’s should be prepared to work across a variety of service hours and able to work independently and autonomously in this role, with a requirement for lone working during nights.
