An exciting opportunity has arisen to join our Same Day Emergency Care team within Whittington Health in London as Clinical Nurse Manager. Come join our dynamic and dedicated multi-disciplinary team.

Same Day Emergency Care (SDEC) is part of the Emergency & Integrated Medicine directorate and provides same-day care to patients referred in from GP’s and A&E. You will work alongside our excellent MDT to ensure patients receive quality, timely and safe care through a bespoke pathway, helping to reduce length of stay and improve patient experience.

The SDEC service aims to provide rapid senior decision making and advanced diagnostics at the front door of the hospital. The role will give the post holder the chance to work with colleagues from acute specialities, the emergency department, primary care as well as community services and as part of an Integrated Care Organisation (ICO), the role will provide a unique opportunity to be part of embedding a new service to provide a truly integrated care pathway for patients across acute and community settings; developing and expanding the services to provide the best care possible for our patients in the best way for them.

Whittington Health is committed to the development of SDEC as part of transformational change. The primary responsibility of the Clinical Nurse Manager is to provide nurse led care to patients being treated on an Ambulatory Care Pathway.

The post holder will ensure that all patients are provided with safe and high-quality standards of nursing care. They will be aware of the Care Quality Commission standards and support the multi-disciplinary team ensuring that they are maintained.

This role encompasses clinical management of a group of patients attending the AEC Clinic. The post holder will have responsibility for the assessment of referrals, care delivery, efficient co-ordination of clinic ensuring smooth running of service and discharge planning. Co-ordinate the flow of SDEC patients within the clinic daily, balancing both planned appointments and on the day emergency referrals

The post holder will directly line manage the nursing team within the unit and provide leadership for nursing within the clinical area and work closely with staff within the AAU, Urgent Care Centre, Emergency Department and Community Services.

Medical services have been delivered on the Whittington Hospital site for over 500 years.   • Provision of specialist, high quality, expert nursing care and support for patients who are suitable for Ambulatory Care in liaison with the multidisciplinary teams (MDT) through assessment, care planning, liaison with others and evaluation.
  • Perform physical examinations and make provisional diagnoses on patients within AEC when appropriate.
  • Initiate and interpret and agreed range of diagnostic tests within ambulatory care pathways

Liaison: • To promote the service and support clinical staff to facilitate the safe, effective and appropriate transfer of patients identified as suitable for Ambulatory care from ED, wards and the communit
  • To work with and be responsible for the case management of patient’s identified for Ambulatory Care.
  • Attend multidisciplinary meetings and participate in cascading information from these meetings to relevant parties involved in the Ambulatory Care pathway proces
  • Record data and support the collection of data for performance monitoring and report
  • Maintain a patient database to monitor patient outcomes and also to audit the effectiveness of the ambulatory care pathway with the aim of developing the service.
  • Understand the protocols and pathways for Ambulatory care and work with the MDT to develop them as the ambulatory care model progresse
  • To advise and recommend appropriate discharge options to patient, family and MDT in relation to any further care needs following discharge such as community nursing.

Management / Leadership responsibilities: • Manage and co-ordination of patient ca Be an authoritative presence within AEC and a role model for all staff.
  • Develop patient condition specific management plans to improve patient clinical management in co-operation with medical colleagues
  • Ensure staff other nursing staff are aware of pathways and procedures relating to ambulatory care. Disseminate any changes in policy and procedures in a timely manner and to work within the
  • To attend appropriate internal and external meetings as representative of the Trust in relation to the Ambulatory Care Nursing role and responsibilities
  • To develop Ambulatory care pathways, associated protocols, and relevant patient group directions to ensure that; patients are suitable for the service are identified as early as possible during their ED visit, inpatient stay, are recruited into the service, assessed and a plan of care agreed.
  • To gain real time feedback from the users of the service to ensure that they are benefiting and that the service delivery is the best possible.
  • Contribute to a summary report and clinical audit of the Ambulatory care service and evaluate its effectiveness on patient outcome
  • Directly line manage all Band 6 and Band 5 nurses as well as Registered Nursing Associates and HCA’s working within the area.
  • Have responsibility to ensure mandatory training is completed by staff and the Rota is completed in a timely manner
Professional responsibilities: • To act within the Nursing and Midwifery Council Code of Conduct.
  • To undertake further educational qualifications and training in order to increase professional knowledge and remain updated
  • To be aware of national policies in relation to Ambulatory Care pathways and service provision.
OPAT

  • To provide support and advice to patients attending the Trust and their families and carers, either as inpatients or outpatients.
  • To act as a resource to other professionals on OPAT and associated treatments and issues.
  • Practice autonomously and be self-directed. To help develop and expand the patient pathway for OPAT, and associated protocols.
  • Ensure suitable patients are identified as early as possible in their illness helping to prevent avoidable hospital admission and decreasing length of stay for those who are already inpatients
  • Undertake assessment of individuals using a range of different assessment methods, which may include physical examination, ordering and interpreting diagnostic tests or advanced health needs assessment.
  • Ensure that appropriate intravenous line management is instituted and integrated within Trust guidelines.
  • Draw on a diverse range of knowledge in their decision making to determine evidence based therapeutic interventions; this may include prescribing medication and actively monitoring the effectiveness of therapeutic interventions.
  • Ensure that appropriate antimicrobial therapy is optimised and delivered using evidence-based practice
Service Improvement
  • To Lead on the expansion and continuous evaluation of AEC services to ensure the unit is working at capacity to off load the emergency department, prevent unnecessary admissions and to facilitate as much as possible the early discharge of patients from hospital beds
  • To work closely with all specialties in the creation and development of pathways which allow for admission avoidance and or the reduction in Emergency Department attendance
  • To work closely with the ACP/ENP team in the Emergency department to ensure the 5% redirection targets are met and were possible improved on
  • To assist on collation of data for the service key performance and ensure alongside ED colleagues that a plan is in place to continuously improve on results as required.
  • To be highly visible in undertaking regular safety rounds and be accessible to nursing and medical staff, patients, and the public, ensuring open and honest communication and examining high standards of care.
  • To work with the Head of Nursing and matron to ensure that care is evidence based and that continuous quality improvement programmes are in place that constantly challenges practice.
  • To improve the quality of patient experience by identifying and meeting the individual clinical needs of patients, including issues pertaining to child protection and safeguarding vulnerable adults.
  • To work closely with the Whittington Health PALS department and where necessary participate in investigating complaints and clinical incidents in line with policy, procedures, and time scales, making recommendations about changes to practice and service that may result.
  • To advocate the collection of friends and family results.
  • To ensure, alongside the Lead Nurse and matron, that the teams follow appropriate and relevant procedures, guidelines, and protocols, working with the Head of Nursing to review these as needed.

EDUCATION & TRAINING
  • To promote a continuous learning environment for the nursing and medical teams ensuring that staff have access to information that enables them to practice safely and effectively.
  • To contribute to the development of nursing knowledge and skills as part of the vision to extend nursing practice within AEC.
  • To teach and support medical staff, student and newly qualified/less experienced ACPs/ENPs, junior and senior nurses, student nurses, paramedic trainees and other staff as appropriate in the clinical setting.
  • To contribute to peer review and supervisory processes.
  • To collate information about the training and development needs of staff as identified through annual reviews and liaise with the professional development team to ensure these are met where appropriate.
  • To ensure where appropriate that arrangements are made for educational evaluation and for staff to learn from the outcomes of quality reviews, audits, complaints, and investigations.
  • To undertake mandatory training as required for the role.
  • To be aware of one’s own continuous professional development needs and maintain an annual personal development plan.
RESEARCH, AUDIT AND DEVELOPMENT
  • To proactively initiate, lead and participate in audit as required and in line with the AEC audit programme, devising audits and presenting findings as needed.
  • To lead on data collection for specific local and multi-centre research projects.
  • To ensure that results of mandatory and departmental specific audits are acted on and changes in practice implemented.
  • To contribute to the development of the Emergency and Urgent Care and wider Whittington Health nursing teams by participating in working parties, committees and planning and discussion groups as agreed with line manager.
