Continence Nurse Advisor
This post will provide clinical expertise within the Continence Service in Enfield. The service is provided to a multicultural and diverse population and aims to deliver a very high standard of care to all patients.
You will be expected to work in partnership with primary and secondary care colleagues and other agencies to ensure a smooth patient journey across the whole patient pathway, from diagnosis to end of life care. Self-care will be encouraged and patients and carers will be actively involved in the development of care plans and the management of their condition.
The post holder will be expected to perform a comprehensive specialist assessment of patients with diverse presentations and complex physical and psychological conditions and develop and deliver an individualised treatment programme.
To hold responsibility for own caseload, supervision takes the form of regular individual sessions.
To undertake all aspects of clinical duties as an autonomous practitioner.
To undertake evidence-based audit and or research projects to further own and team's clinical practice with support and guidance from more senior staff.
Make recommendations to the clinical lead/manager of service for changes to practice by the team. May contribute to the implementation of specific changes to practice or contribute to service protocols.
Be aware of and highlight to your line manager any clinical governance issues pertaining to your role and practice.
To contribute to the delivery of health promotion activities within the service and wider trust initiatives.
