CONTRACTUAL COMMUNITY HEALTH WORKER
POSITION OVERVIEW
The Community Health Worker(CHW) promotes and supports Care Management processes and quality initiatives. The CHW supports the care coordination and care management of Medicaid members through varying responsibilities facilitating the flow of information relating to individual client-members including, but not limited to those listed.
ESSENTIAL POSITION RESPONSIBILITIES
- Find Hard to Reach (HTR) and/or Unable to Reach (UTR) client-members via being provided a list with names, addresses and phone numbers
- Work with the team to support safe transitions of care and connections to social determinants of health services and resources
