Diabetes Nurse Practice Facilitator

Bromley Healthcare is a key partner delivering diabetes care within One Bromley Integrated Care Partnership.
An exciting opportunity has arisen for a dynamic, motivated band 7 Nurse to work within our diabetes team, who can help shape and improve the diabetes 8 care processes and 3 treatment targets delivery and achievement.
This 1 year fixed term role, is non-patient facing and is focused on developing, embedding and improving our processes and practices.
Our ideal candidate would be expert in these areas of diabetes care, have education and training delivery experience and be comfortable working with and communicating with clinical and non clinical colleagues across organisations.
As this post is directly supporting the improvement of the 3 treatment targets for diabetes (and 8 care processes), the role remit may evolve as the project develops over time.
The primary function is to support Primary Care Networks in improving achievement of the 3 treatment targets associated with cholesterol, Hba1c and blood pressure and the wider care processes.
The post holder will work along side and be supported by the Multidisciplinary Diabetes team including Consultant, DSN's and specialist Diabetes Dietitian based at Beckenham Beacon
Bromley Healthcare is a community interest company providing a wide range of services including community nursing services such as district nursing, health visiting to specialist nurses, as well as therapy services for children, young people and adults.
Salary is dependant on NHS experience and current banding/pay point.
