Housing Community Health Worker

This position will be an integral part of a dynamic team and is responsible for outreach into the Hispanic/Latinx community while also working with current clients.

The Housing Community Health Worker is a full-time, hourly position at Rebuilding Together South Sound (RTSS) and is supervised by the Director of Programs (DoP). The HCHW’s primary responsibility is to perform outreach into the Hispanic/Latinx community while also working with RTSS’s current clients on promotion of respiratory health, management of chronic lung disease and falls prevention. The role of the HCHW is to address health disparities and foster a relationship of mutual respect between our clients and our program.

Duties and Responsibilities

Home Visits and Client Education (60%)

  • Provide client education and home visits related to lead based paint, asthma, COPD, or other healthy homes concerns in order to improve health outcomes and quality of life.
  • Provide support to programs team on initial home visit for client requested repairs including completing 25 safe and healthy housing priority checklist and providing additional resources to clients.
  • Assist in the development of program strategies, community-based outreach, and education activities.
  • Accurately enter data and prepare reports for program evaluation using Salesforce and Microsoft products.
  • Provide on-site/in-home assessments for safety risk and health needs.
  • Use motivational interviewing and other behavior change strategies to support client behavior change to improve safety and health.
  • Follow up with clients via phone calls, e-mail, or home visits.
  • Provide support for monthly volunteer projects for 1 volunteer project per quarter including Rebuilding Day in April, Rebuilding a Healthy Neighborhood and RampUp in October.

Outreach (35%)

  • Work with RTSS staff, community partners and advisory groups on an Equity Outreach Strategy.
  • Seek additional ways to outreach into the Hispanic/Latinx community to expand awareness of community resources, services, and programs (including Rebuilding a Healthy Neighborhood).
  • Assist homeowners and community partners by providing appropriate promotion materials or referrals to community resources and services.
  • Assist in the development and marketing of health promotion materials related to lead-based paint, asthma, COPD, or other healthy homes concerns.
  • Support community coalitions.
  • Conduct presentations and classes.
  • Attend outreach events.

Programs Administration (5%)

  • Participate in staff/board activities and Rebuilding Together Inc or partner work groups as relevant/requested.
  • Help ensure the smooth operation of the office and other shared duties as assigned.
  • Support fundraising activities as requested.

Essential Job Functions of the Donor Development Manager position include, but are not limited to:

  • Computer proficiency with Microsoft Office products as well as experience with database systems, preferably Salesforce

Qualifications and Required Skills

  • Bilingual (Spanish) required
  • Minimum Associate’s degree and one year of experience in a community health, medical office, community-based organization, OR three years’ direct experience.
  • Community Health Worker Certificate preferred.
  • Must reside in or be very familiar with the language and culture of the Pierce County and the Hispanic/Latinx community being served.
  • Local community involvement, experience with home visits, health-based or environmental outreach and education, and experience with asthma, COPD or healthy home concerns is preferred.
  • Maintain CPR/First Aid certification and any other certification requirements.
  • Must have a vehicle, a valid driver’s license, and proof of insurance.

Workspace: Initially, the job requires a minimum of 80-100% in-office presence during regular business hours. After successful completion of a 90-day probation period, the job will require 40-60% in-office presence during regular business hours. Almost all work will take place during the weekdays, although occasionally there is an evening or weekend event or meeting. RTSS believes in a flexible working environment and hours and will collaborate with the HCHW to plan a schedule that works for all.

Compensation & Benefits: Salary range is $48,000 - $52,000 per year. Compensation will depend on experience, demonstrated skills, and relevant past success. RTSS offers a generous benefits package that includes medical, dental and vision benefits and 401(k) retirement. Medical and dental insurance are offered through Regence Blue Shield. Employees enjoy flexible scheduling and a culture of care that values taking time away from work – including 9 paid holidays (4 of which are flexible days) and a paid office closure every year between Christmas Eve and New Year’s Day. We offer 10 days of accrued PTO for new employees with PTO increases at 3 and 7 years.
