Oral Health Community Health Worker

Shawnee Health Service is seeking an Oral Health Community Health Worker for our Carbondale and Marion Dental Clinics in Jackson & Williamson County. The Oral Health Community Health Worker will have a variety of responsibilities including:
  • Working within the communities of Jackson and Williamson Counties, to identify target populations, working closely with primary care teams and other agencies to improve patient care outcomes, close care gaps, and assist the patients in navigating the health care and social service systems.
  • Engage in the community through outreach activities and events.
  • Serve as liaison between Medical healthcare, oral healthcare and social services and the community.
  • Provides health education through the utilization of outreach activities, informal counseling and social support.
  • Supports the patient/client in development and implementation of health management plans and goals.
  • Coordinates services between providers; including scheduling appointments and transportation.
  • Assesses the needs of the patient/client and assists in completion of applications for programs/services which they may be eligible.
  • Provides coaching for patients/clients on effectively managing their oral health conditions.
  • Run data reports as needed to meet program requirements and maintain surveillance of the project.
A High School Diploma or GED is required. Some evening or weekend events/meetings are required. Position requires both remote work and in office work.
COVID-19 vaccination is a mandatory condition of employment. Absent an approved exemption/accommodation, all employees must be fully vaccinated by receiving both doses of a two-dose vaccine series (Pfizer or Moderna) or one dose of the single-dose vaccine (Janssen).
