Job Description:
Community-based position working for Healthy Start program. Community Health Worker / Parent Educator will provide direct support and assistance with resources to mothers, infants, and their families with the goal of improving health status outcomes before, during, and after pregnancy, and reducing racial/ethnic differences in rates of adverse perinatal outcomes.
Ideal candidates:
- Possess an Associate's or Bachelor’s degree in social work, psychology, or a related field. Minimum of 2 years related experience can substitute for a degree. (Either education or experience is required.)
- Schedule is generally M-F, 8am-5pm with occasional evening and/or weekend flexibility.
- Bilingual preferred. Minorities are encouraged to apply.
Responsibilities include:
- Provide parenting education utilizing evidenced-based treatment models and curriculum;
- Provide case management and coordination of referrals and services;
- Provide health education and promotion activities;
- Complete participant enrollments and related assessments;
- Develop, implement, and monitor individualized treatment plans;
- Create a culture of respect including awareness of ethical, culturally-sensitive, and trauma-informed interactions;
- Participate in regular supervision on clinical and administrative issues;
- Maintain timely and accurate clinical records in accordance with organization policies;
- Maintain effective working relationships with clinical team and community referral sources;
- Maintain necessary communications and reporting with referral sources and other involved parties as approved by client; and
- Complete other duties as assigned.
Must be able to travel extensively. Position can be based in any of the following Indiana counties: Fayette, Henry, Jackson, Jefferson, Rush, Scott, Union, and/or Wayne. In-person travel is required across Jackson, Jefferson, and Scott counties. May include overnight stays.
