Hourly Rate: $17.66
Position Summary:
Full-Time Pathways Community Health Worker position available immediately for Everett's Care Management Program. The Pathways Community Health Worker must be willing to travel throughout Snohomish and Skagit County.
This position requires the CHW to be able to drive to meet clients, flexibility, self-initiative, and the ability to manage time, resources, and client caseload efficiently. Under the direction of the Pathways Supervisor, this position works closely with all clients, community organizations, and other departments. This culturally and geographically connected individual serves as a link between underserved communities and existing community resources. Through home visiting, this individual assists clients in overcoming barriers to health, social services, education and employment.
Qualifications and/or Education:
- High School Diploma or Equivalent required.
- Bilingual required.
- Experience working with people from various backgrounds required.
- Experience working with individuals with substance use disorders, homelessness, chronic mental illness, or people in crisis preferred.
- Prefer one year paid or volunteer experience providing medical, social service or behavioral health case management or care coordination.
