The Bridge, an innovative and well-respected mental health agency serving adults with serious behavioral health diagnoses, substance use issues, and co-occurring medical conditions is seeking a Full Time Psychiatric Nurse Practitioner to share time on an Assertive Community Treatment (ACT) Team and a Forensic Assertive Community Treatment Team (FACT) both located in the Bronx. The Psychiatric Nurse Practitioner is a member of an interdisciplinary team that provides clinical services to adults with serious behavioral health diagnoses, who have histories of multiple hospitalizations and who have not been successful in traditional office based treatment settings. unities we serve. This is a field-based position. 80% of the work is conducted in the field.

Responsibilities:

• Conduct Psychiatric Assessments and Evaluations;
• Prescribe, Monitor and Administer Psychotropic Medications;
• Provide crisis intervention services to clients during regular business hours and consultation during off-hours;
• Participate in clinical case conferences, team meetings, High Risk conference, quality improvement, evidence-based practice implementation and policy/procedure development

• Assess and monitor the physical health of clients; complete all clinical documentation as required by payers, funders, and regulatory authorities via an Electronic Health Record; complete all administrative paperwork required

Qualifications: Board Certification Psychiatric Nurse Practitioner either with appropriate experience or a strong desire to work in this rewarding setting. New graduates are welcome to apply. Bi-lingual Spanish highly desirable.
